The Primary or Secondary Progressive Stages of Multiple Sclerosis
When a new relapses occurs, an increased level od a fixed disability will be come challenging. The patients self-image is forced to readjust, the unpredictable tempo of Multiple Sclerosis goes on.
The natural history evaluation of large populations is followed over several decades shows that within 15 years of the first onset of the dease of MS almost 50 percent of the people will require a full-time walking aid such as a cane or a crutch. Many of the patients may have to contend with other impairments as well such as: decrease vision; distressing problems of poor bowel and bladder control; muscle tightness (termed spasticity); besides being poorly understood but with common fatigue, along with weakness, clumsiness. Also there is the tremors in an arm or leg, and posibly a deccline in intellectual functions (partial dementia). The patients understandably develop various levels of depression as their loss of daily funtion becomes evident an increases over time.

When MS goes into a relapsing phase during which the patients experience a relapses followed by some little recovery that is separated by unpredictable intervals of MS and than sometimes sooner or later you are back to normal.
When this course and pattern of MS changes the patient experience an increase or a progressionin neurologic disablity. Which may advance quite rapidly or may come on slowly over a period of several years. During this time it maybe interrupted by plateaus of relative neurologic stability.
The patient may look very normal on the outside as if to look very Healthy.
The term Secondary Progressive Phase is used because it is progressive (increasing disability to the patient) but is secondary to the preceding relasping-remitting phase of MS.
Some Patients have describe how one year they are able to walk and go up stairs in their home with no problem. But months later they have to have help getting up the same staris. They have to use the banister to help them get up the staris.Some patients have to get a chair lift to manage this task.
Some patients have to discuss this with family and caregivers as to how to proceed if they want to live in their home.

James Eckburg and MS: CIS(Clinically Isolated Syndrome)
Now after the first episode of neurologic sympstoms an individual in their life is to have MS your physician calls it the clinically isolated syndrome. So without a history of your repeated relapses or other neurologic problems it is impossible to know for sure if you have MS. With that history of your characteristic adnormalities you have show up in your brain. That is when your doctor will have you get an MRI to check to see if you have any dark spots on your brain leassions that will confrim that you have MS but you can still have MS and have no dark spots on your brain also. In that case your MS could be in your spinal coulum. That can be check with a spinal tap taken to check your spinal fluid.
Back in 2010 there were many carefulley constructed placebo-controlled clinical trailsto see if using currently avaiable MS treatments show an active theraphy during recovery period from the CIS that will delay the occurance of the next MS relapse in most people.Your doctor takes a look at all of the scientific data that was done and is clear the doctor will have a medical recommendation of a therophy to start for you, it is difficult know how you are going to react becasue after you start you might start to improve and be back to normal some what. That is hard when you doctor asked you to start a treatment that medication that must be given by injected frequentlty, or daily to weekly is often might have side effects. You might have to have this treatment for the rest of your life.
Now we will talk about the Second Clinical Attack or Relapse:
When a person gets a second clinical attack,you may think that it is similar to the first attack. It may more than likely that the disease typically reappears in another part of your body in the nervous system. The new symptons are new but they aeffect another part of your body. You may have trouble with unilateral vision if the previous attack had been in the spinal cord.
Some patients the second attack could be the one attack that will change your life. I had several years betweenmy attacks that with the treatment my Dr. gave me I was able to go back to work after my 10 days in the hospital for treatment. My treatment was called ATCH. You had to go to the hospital and stay it was put into your body through your viens. I had have several of these treatments that were spaced over years. The last treatment was my last time that I was able to walk into the hospital back in 1989. I have been in a wheelchair ever since.
Dr. Rosalind Kalb, who was head of the Professional REsource Center at the National Multipile Sclerosis Society in New York and he also is a psychologist has writen a note that will give a perceptively about the emotionally devastating nature of getting the second attack:
"I guess I really do have MS"
"The doctor was right after all"
"I'm going to end up in a wheelchair"
"Life will never be the same"
"No one will want me "
"There go my dreams for....."
That might be true but YOU can rise above that and be able to go on with your life as before. I have said that "I have MS and that MS does not have ME". I have my down days that is true. But if YOU wake up in the morning and "SMILE" before YOU get out of bed. Than YOUR day will be a good one.
I have often thought of how Job felt in the Bible. I could not do the things I did before. Like walk outside in the yard. Play ball with my son. I have two children and my daughter is the oldest and she can remember her dad walking but my son has not seen his dad walk. That is the hardest thing I know. I have a nother verse in the Bible that I go to and that gives me the strength more than any meds. That Bible verse is Philippians 4:13(I can do all things through Christ which strengtheneth me. (KJV).
